Foundation4Love is a Nonprofit who works with New England Cancer Specialists providing services to their cancer patients. In 2022, we proudly announced our expansion in services, working with Boston IVF in Maine and New Hampshire to help financially support Men and Women eligible in Fertility Services.

Who qualifies for assistance?

In order to be eligible for financial assistance, you must:

• Be an active patient at New England Cancer Specialists.

• Be a patient who is undergoing chemotherapy for cancer.

• Demonstrates financial need. Please see eligibility requirements below:

Household size: INDIVIDUALS: $60,000 or less 

Household Size: COUPLE: $100,000 or less 

Household Size: 3 OR MORE: $150,000 or less

• Use infertility services at Boston IVF in Maine
   

PLEASE NOTE: We will provide assistance on expenses once your application (below) has been approved.

When will I be notified?

You will be notified within 48 hours of submission of a completed application regarding the status of your application. Foundation4Love will contact you and Boston IVF to make them aware of your eligibility. It is at that time that Boston IVF will reach out to you directly OR if you do not hear from them in 3 business days, please contact their Maine office, 207-358-7600. If you have not heard back, please contact us by email at info@foundation4love.org.

Amount of assistance and how I will be reimbursed?

FOR MALES undergoing sperm banking:

If eligible (see above), the total assistance amount that will be available to you from Foundation4Love will be $1,250.

Breakdown of cost:

- X1 sperm Freeze = $350.00

- Sperm Storage for 9 months = $900.00

- Total Assistance given by Foundation4Love: $1,250

• Foundation4Love will assist patients who are financially eligible with a one-time sperm freeze ($350.00). If the patient chooses to do more than the one-time freeze there would be additional charges from the patient payable to Boston IVF.

• Foundation4Love will pay for the 100$ monthly cost for storage fees up to ONE YEAR. Please note that storage charges after 1 year from the date of freeze will no longer be paid for through Foundation4Love and will be the patient's responsibility.

FOR WOMEN undergoing fertility preservation.

Foundation4Love will pay up to $5,000 towards women receiving treatment for fertility preservation.
